NOTE: Patients over age 65 are welcome here. We are opted out with Medicare. We do not take any Medicare plans. If you are a Medicare beneficiary, we will arrange a self-pay, private contract. All patients over age 65 must sign an affidavit to their Medicare status.
Insurance we take: First Choice, United (not United-Apple), Aetna, Cigna, Premera, Regence, Kaiser PPO, all blue cross. We do not accept Medicare, Medicare Advantage, AARP, Medicaid, Apple, Molina, Tricare, Humana, Kaiser core. Our self-pay prices are $120 for new patient exam. Most procedure prices are $100-$300.
